What is lymphedema and how is it treated?
Lymphedema is not a condition that must be managed for life with bandaging and massage alone. It is a disease that can be treated microsurgically from its early stages. This page is written to answer your questions on staging, diagnosis, surgical options and recovery.

What is lymphedema and why does it occur?
The lymphatic system is a fine network of channels that collects protein-rich fluid from the tissues and returns it to the circulation. When this network becomes blocked or damaged, fluid accumulates in the tissue; the resulting persistent swelling is called lymphedema.
Main causes
- Secondary lymphedema: develops after removal of lymph nodes during cancer surgery, radiotherapy, recurrent infection or trauma. This is the most common form; arm lymphedema after breast cancer is the typical example.
- Primary lymphedema: caused by congenital underdevelopment of the lymphatic system. It may appear at birth, during adolescence or in adulthood.
Over time the swelling ceases to be merely a matter of appearance: fat deposition and fibrosis develop in the tissue, the skin thickens, recurrent episodes of cellulitis begin and movement becomes restricted. Timing therefore directly determines the treatment options available.
The response to microsurgery performed in the early stage, while lymphatic channels still function, is markedly better than that achieved once tissue hardening has set in. Waiting narrows the options.
Lymphedema or lipedema?
The two conditions are often confused, yet their treatments differ. The table below offers a guiding framework, but a definitive distinction can only be made through examination.

The two can also coexist. Untreated lipedema increases the lymphatic load over time and can lead to a combined picture known as lipo-lymphedema. Details are on our lipedema surgery page.
Stages of lymphedema
The International Society of Lymphology (ISL) staging system is the framework that determines which surgical option is appropriate.

The general pattern: in stages 0–2, LVA comes to the fore to the extent that lymphatic channels are preserved. In stages 2–3, vascularised lymph node transfer and debulking surgery are planned alone or in combination. The final decision is made together with the imaging findings.
Diagnosis and lymphatic mapping
Surgical planning cannot be done without seeing the actual state of the lymphatic system. The principal methods used are:
- ICG lymphography: indocyanine green dye injected under the skin is tracked with an infrared camera. It produces a real-time map of functioning lymphatic channels and is used to determine where LVA incisions are placed.
- Lymphoscintigraphy: assessment of lymphatic flow and nodal involvement using a radiolabelled tracer. One of the reference methods in staging.
- MR lymphangiography: a detailed anatomical map of lymphatic channels and fluid distribution. Preferred in advanced stages and complex cases.
- Clinical assessment: circumference measurements, the Stemmer sign, skin quality, history of infection and response to previous treatment.
Surgical methods
Lymphedema surgery divides into two philosophies: physiological methods that re-establish lymphatic flow, and debulking methods that remove accumulated tissue. In many patients the two are used together in stages.
Lymphaticovenous anastomosis (LVA)
Blocked lymphatic channels are connected under the microscope to adjacent veins roughly 0.3–0.8 mm in diameter, redirecting lymphatic fluid into the venous circulation. It is usually performed through incisions a few centimetres long, is the least invasive physiological option in early-stage disease, and requires only a short hospital stay in most patients.
Vascularised lymph node transfer (VLNT)
A package of lymph nodes is taken with its blood supply from a healthy donor site, transferred to the affected region and connected microsurgically. It is preferred in advanced stages and where lymphatic channel damage is widespread; it can be planned at the same time as breast reconstruction.
Debulking surgery
Removal of accumulated fat and fibrotic tissue using liposuction techniques that spare lymphatic structures. It markedly reduces volume and weight in advanced stages; compression garments are required continuously afterwards.
Preventive approach (LYMPHA)
Lymphaticovenous connections created at the same time as lymph node dissection during cancer surgery. The aim is to reduce risk before lymphedema appears. It is planned together with the oncological surgical team.
No single method is right for every patient. Even in two patients at the same stage, lymphatic mapping may call for a different plan. The choice of method follows a combined assessment of stage, imaging, previous treatments, skin quality and expectations.
Surgery and recovery
- STEP 1 — FIRST CONTACTInitial contact via WhatsApp or the form. Initial guidance based on duration, previous surgery, radiotherapy history and any images you share.
- STEP 2 — EXAMINATION AND IMAGINGCircumference measurements, clinical assessment and, where indicated, lymphatic mapping with ICG lymphography or lymphoscintigraphy.
- STEP 3 — PLANNING CONSULTATIONApplicable methods, expected benefit, risks and the number of sessions are discussed openly. The decision is never rushed.
- STEP 4 — SURGERYIn a fully equipped hospital setting. LVA usually requires only a short stay; lymph node transfer and debulking surgery involve longer admission.
- STEP 5 — FIRST 6 WEEKSCompression garments, wound care and gradual mobilisation. The first changes in swelling begin to be noticed in this period.
- STEP 6 — 3–12 MONTHSResponse is monitored through scheduled reviews and circumference measurements. The intensity of lymphedema physiotherapy is adjusted gradually.

Frequently asked questions
Who is suitable for lymphedema surgery?
Candidacy is determined by stage, the state of the lymphatic system on imaging, how long the swelling has been present, skin quality and general health. In early stages (ISL 0–2) LVA comes to the fore, while in advanced stages lymph node transfer and debulking methods are considered. Active cancer treatment, uncontrolled infection and advanced vascular disease may alter the timing.
Will I still need to wear compression garments after surgery?
Most patients continue to use compression in the early period. The duration and pressure level can be reduced over time; whether they can be discontinued entirely depends on the stage, the method and the response achieved. Reduced dependence is reported after LVA performed in early-stage disease.
How long does recovery take?
After LVA most patients return to daily life within a few days. Recovery is longer after lymph node transfer and debulking surgery. The reduction in swelling is gradual rather than sudden, occurring over weeks and months; this is why follow-up measurements matter.
When should I seek help for arm swelling after breast cancer?
You should be assessed without delay when you notice an increase in arm circumference, a feeling of heaviness, or rings and watches becoming tight. Microsurgery performed early is far more effective than treatment after tissue hardening has set in.
Can lymphedema resolve without surgery?
Complex decongestive physiotherapy, bandaging and compression garments help keep swelling under control and are an indispensable part of treatment. However, they do not remove the blockage in lymphatic flow. Surgery is a complementary step that targets the underlying drainage problem.
Will my episodes of cellulitis become less frequent?
Accumulated lymphatic fluid increases the risk of infection. Improved drainage is reported to reduce the frequency of recurrent cellulitis. However, this outcome varies by individual and stage and cannot be guaranteed.
Both my legs are affected — is it done in one session?
This depends on the extent of the affected area and the planned method. Large areas are usually divided into more than one session for safety and healing quality.
Are lymphedema and lymphoma the same thing?
No. Lymphedema is a circulatory problem caused by lymphatic fluid accumulating in the tissue. Lymphoma is a cancer of the lymphatic system, treated by haematology and oncology. The similarity of the names often causes confusion.
